b. Move your father to a new nursing home (after any hospital visit);

I have successfully handled cases like this. Nursing homes are required to assess you father's risk of falling and prepare an appropriate comprehensive care plan. Illinois and Federal law require nursing home to "provide the necessary care and services to maintain the highest practicable physical, mental, and psychosocial well-being, in accordance with the comprehensive assessment and plan of care."
You need to retain an attorney to investigate whether the nursing home fulfilled its obligations. In my experience, nursing homes are often understaffed and pursue profits over safety.

This matter could very well be a claim worth pursuing. I am presently handle at least two similar cases. You have omitted many facts about how the incident occurred. But I would recommend tthat after your father receives proper medical care, that you consult with a medical malpractice, nursing home abuse or personal injury attorney.
I assume this was a private nursing home. If not then governmental claims filing statutes would apply. Such claims filing statutes are very short, some as short as 90 days. Therefore obtain a consultation as soon as possible.
