If a offer of judgment has the attorney listed as, "for the Plaintiff" on the top but you are the plaintiff is it valid in court

If I am going to accept this offer I want the paperwork to be correct but below the defendant's attorneys address it says Attorney for the Plaintiff.

You want to check with an attorney to determine if it's a good offer. Most give free consultations.
Don't ever bank on getting something thrown out based on a thing like that. An offer of judgment can ONLY be made by the defendant. If a defendant makes an offer of judgment, and then you are awarded less than the amount of that offer at trial, the defendant can recover his/her fees from you. I don't see a judge saying "you don't get your fees because you listed yourself as 'for the plaintiff' on this document." There is a myth out there than any typo will get your charges dismissed or an issue decided in your favor. Don't believe it.
