How is this completed? Is there a form I can fill out and file it with the court and it become part of the record?

Proper preparation of a Warranty Deed requires very specific legal training and experience. First, I would advise against attempting to prepare these documents yourself. Second, customarily, and unless your Divorce papers say otherwise your ex-wife's attorney would/should typically prepare those documents for you to review with your counsel, execute/sign and return. (along with the corresponding transfer documents) But I would definitely refer to your Divorce papers and be guided accordingly.
Actually, there are several forms. You need to have a deed drawn up and transfer tax returns and then the papers have to be recorded in the County Clerk's office. It is best if you have an attorney prepare and file the paperwork.
There are other real property forms that must be prepared in addition to the deed in order to file the deed. If you want it done right and without headaches, you should hire an attorney to do so. Most attorneys would only charge a nominal fee for this type of transfer between spouses. You should consult with an attorney in your area.
Why not retain the attorney who did the divorce?
He has the most information regarding your case and doesn't have to be brought up to speed.
