Help Filing out USCIS FORM I-90 for renewal (2 of 3)

I am helping my grandma filling out USCIS Form I-90, but I have a couple of questions. I have the application filled out and will E-Filled. We live in Texas. Any help is greatly appreciated! In part 3, No. 3: Location where you applied for an immigration visa or adjustment of status; If a visa was not used to enter, but an application for Permanent Residence was submitted once here, what would go here; and would this need to be a specific office location or a city and state should suffice? 5.) In part 3, No. 4: Location where immigrant visa was issued or USCIS office where adjustment of status was granted; Again, if a visa was not used to enter, but an application for Permanent Residence was submitted once here, what would go here; and would this need to be a specific office location or a city and state should suffice?
