I need help answering part 3 of form I-90 to renew my green card.

My mother petitioned me and my siblings to get a greencard. I remember then that I had been interviewed at the consulate in Manila before coming to the US. My class of admission is E35 (child of skilled worker). Other than that I can't remember anything else. Question 1: location where you applied for an immigrant visa. Is this the location from where my mom applied to petition us? or Manila? Question 2: location where immigrant visa was issued. Is this Manila? Question 3: at the certify forms page: do i not fill up preparer certificate form since I personally filled up the form?
