Can you mark more than one box in the FL-180 form?

This is a default with an agreement..The Law Facilitator told me to fill out the FL-341 and FL-342 and attach it along with the written agreement..My question is can I mark 2 boxes in the questions 4k and 4j on the FL-180 form? example for 4k can i mark #1 and #2?
