Notice of appearance form

Just copy the caption, put "appearance" where it now says "complaint" (or answer or whatever) and write: [Your name] hereby enters his / her appearance as pro se counsel as to all issues in this litigation. And print and sign your name. That's all you need. Don't stress it.
