I need to do a new Sworn Financial Statement, do to a change in income and Employer. Do I do a whole new statement on my financial income? same Document/Form as before?
Also shouldn't the other party call Mediation Service's to set a mediation date? Not me?

Yes, an updated financial statement is the same as the original form.
Whichever person wants to relocate has an incentive to take the lead on setting up the mediation. They are the person who needs to show that they did everything possible to comply with the Court's order. If the other person won't cooperate with mediation, then I would expect you'll be asking the Court to move forward anyway. You want to make sure there is no question that you did everything you were supposed to do.
If you do not have an attorney for a relocation hearing, your chances of success are diminished. At least consult with a family law attorney before the hearing to understand how the process works. Good luck to you.
You ABSOLUTELY need an attorney for a relocation case. As a former judge in CO, it is an uphill battle for a judge to grant a relocation. Most judges want you to have an attorney and a CFI to look at the pros and cons of staying put v. the pros and cons of moving.
