My brother got a underage dui at the beginning of feb 2015, it is now april 2015 and he has yet to turn in his paper license or request an mva hearing. What could happen after the 45 days?

His license will automatically be suspended on day 46 and will remain suspended for 45 days to a year depending whether he took the test and the result. When the suspension is over, he can get his regular license.
The police officer took his regular license already.
He needs a lawyer to defend the DUI charges. If he is convicted of a DUI, he will face a mandatory interlock since he was under 21. He needs to be represented since he faces jail time and the loss of his license.
He will be suspended for some period of time depending on if he refused the test or took it and the result. Depending on the ultimate outcome in court he can be suspended again or even revoked. Obviously, he is also facing jail and probation. He needs a lawyer months ago
After the 45 day mark, his license will automatically be suspended. After the suspension period he must go to motor vehicles to get a new license. He will remain suspended until he gets the new license. Of course, he is talking to a lawyer about defending against these charges.
His license expired on the 46 day after his arrest. He is now suspended and shouldn't drive. At this point he cannot request an MVA hearing. He may still be able to opt for the interlock depending on whether he blew or not and what the number was.
He doesn't need to turn in the paper license. It simply expires on the 46th day, and is no longer a valid license. His actual license was taken by the officer and mailed to the MVA and destroyed. The suspension period is explained on the back of the paper license. Once the suspension period expires, he can go back to the MVA to get a new license, but he'll remain suspended until he does so. He won't get the old one back. If he can afford a lawyer, he should get one, because this is not the end of his MVA troubles. He can apply for a public defender for the DUI court trial, but the PD won't represent him at the MVA.
