When applying for US Passport Form DS-11, What should be mother's name? FullName at birth or use just LastName at birth?

I am a US Citizen and want to apply for US Passport. In DS-11 Application form section 10. Parental Information is asking for Mother's Name. Whats the right name? 1. Mother's FullName at birth? i.e Mothers' name + Mothers' FatherName as middle name and Maiden LastName as last name. Or 2. Mother's Name + Mother's HusbandName as middle name and Mother's Husband LastName as last name. Or 3. Mother's Name + Mother's HusbandName as middlename and Mother's Maiden LastName as last name. Please let me know. Thank you for your time and help.

You cannot go wrong either way. I would write in your mother' s current first and last name, followed by her maiden name, example: Jane EDWARDS, née WILLIAMS ("Née" means "born" )

Unless the instructions say otherwise, I would use her current name.

It is your mothers legal name now or when she past. That could be her birth name or her married name, she may have legally changed it to something else.
