Green Card / Adjustment of status (Birth certificate affidavit )

I am adjusting my status for green card. If both parents are alive are both parents required to present an affidavit for the birth certificate do can one parent and another relative present affidavits. I am not aware of the location or whereabouts of my biological father can my mother and other relatives sign affidavits on my behalf. I am doing this because my birth certificate was entered after I turned 21. This will be used for an adjustment of status to green card. Do the affidavits need to be notarized? Thank you!

As stated, you well need to go to the reciprocity table because not ask countries have the same requirement. You will have to get a document from the government stating there is no record of your birth. In the affidavits to have the declarants state they don't know the status or location of your father. It is a good idea to contact an attorney, these cases can be tougher than they look. Good luck.

If the birth was not registered, the reciprocity chart will guide you with documents you should provide to support your application. Usually it is the non availability certificate supported by 2 affidavits.
