Do I have to report a barring from a supermarket when applying for stuff? Will it be on my permanent record?

I was accused of theft and asked to sign a document stating that I would be barred from the premises for 5 years and that if I entered the premises I could be charged with trespassing. I also had to provide an ID and credit card along with me SSN.

That's a private matter and does not create a criminal record in the usual sense. There's nothing to disclose.

No, since this was not part of the criminal process. You were not found guilty of a crime and did not plead guilty to a crime. The authorities appear not to even have been involved.

If you were charged with theft, the charge may already be on your record. You were charged with theft if you went through a criminal process for that theft. If you were simply accused by a supermarket of stealing, then this is not on your criminal record. However, they will be able to file charges for trespassing and you can be criminally prosecuted if you ever enter into that store again. If you were not charged with a crime then you don't have to report it and it won't be on your record. If you were charged with a crime even if you were not prosecuted, this may show up on your record. Whether or not you have to report a charge is a different story. It is good to know whether or not you were charged with a crime.
