This factual scenario appears troubling, but there are a number of questions that would need to be answered by a local lawyer in his area who is experienced in personal injury law. I suggest your cousin contact such an attorney in Philly so that he can have a free consultation and discuss the specifics of this complicated situation.
The only other source of financial compensation appears to be the possibility of a lawsuit against the used car dealer and/or the driver if the driver was working for the dealer at the time of the accident. If you have an attorney, the attorney should fully investigate that issue before entering into a settlement with the insurance company for the used car dealer, otherwise it will not be possible to bring a lawsuit later. Also, it must be determined whether or not the other driver and/ or used car dealership has sufficient assets to make it worth suing him. At a minimum, an asset check should be performed. Best of luck to your cousin with his case and medical recovery.

Besides the dealer your cousin would have a claim for bills under the crime victims compensation act if the other driver drunk or cited with a criminal offense; I would assume he's applied for social security medicare. Unfortunately, all you can get is whatever the level of coverage is--take a look at your own policy and you will see that you selected an amount. Those amounts, put together, are all there is--even with the dealer there is a set amount of liability.
Aside from the insurances, the dealer themselves could be liable as an owner/employer of the at fautlt vehicle. Don't give up so easily, hire an attorney to explore all sources of recovery
