I paid doc $1600. He has paid nothing. He said he won't pay me until ins. pays him, which might be never because he is so far out no money. I have spent $15,000 already on legal fees and I am broke.

I'm sorry that this process isn't working for you. I'm afraid it only works, or works best, when the two parties cooperate in good faith. If one party wants to jam up the other, he or she can do do. The best you can do is file a motion for a rule to show cause. You take all your evidence to the court and explain how the other party is not complying with an order. You can ask for attorneys fees and the judge can grant them (don't bank on it). Of course it helps to have an expert represent you.

You should consult with an attorney to see whether you already have a court order requiring him to pay a certain amount. If you have been to court already & a judge has ordered him to pay you by a certain date, it may be that DCSE can help collect the medical reimbursement for you. It may also be that an attorney can advise you as to how you can enforce your current court order(s). **This is not intended as specific legal advice nor does it create an attorney/client relationship**
