This is for our dad's estate. 2 adult siblings. My brother is executor. Are we supposed to fill in our info on the "child" section? Or should he fill out the "legal representative" section, because we aren't minor/dependent children?

You should include all of your dad's siblings in the children section, not the legal representative section. As long as all living relatives are included the previous sections, I would leave section five blank.
