Do I have to file a UCCJEA form when filing for the SUPPLEMENTAL PETITION FOR MODIFICATION OF CHILD SUPPORT.

This is a Florida family court in collier county. Two years ago the paternity case reached a final judgment and it had UCCJEA filed. Now I am filling a SUPPLEMENTAL PETITION FOR MODIFICATION OF CHILD SUPPORT and serving the other part by sheriff's office, so I am wondering if UCCJEA is need it again? If the answer is YES - will this mean that it needs to go along and be served with the initial 12.905b or can it be filed with the clerk a week later when the part has already been served?

I think it is required if some of the information listed in the old one has changed. You may serve it with the Supplemental Petition or file/send it later.

The UCCJEA Affidavit is only required by statute when you are seeking to modify the custody, parental responsibility, time sharing or parenting plan that was ordered in the final judgment. If you are only seeking to modify the child support provisions of the final judgment, without seeking to make any change to the existing orders for parental responsibility or time sharing, there should be no need for a new UCCJEA Affidavit. . . . unless the local rules of your county require a new UCCJEA affidavit. If the court clerk can't or won't tell you if the rules call for it, you should call a local family law attorney for a short consultation just to make sure AND to make sure that there are no other requirements that you might otherwise be overlooking.
