The current version of the form G325a asks about the last 5 years of employment and has 5 lines where you can fill out all the employment related data. I have had more than 5 jobs in the last 5 years, am I required to include an additional sheet of paper?

You can use an additional sheet to provide the required information. Provide your name and A number if any on the additional sheet.

Yes, and extra sheet is what you need, but make sure it is clear that it is an attachment to the G-325A, with your name, and date are on it.
