Starting June 1, 2014, USCIS will limit the validity period for all Forms I-693, Report of Medical Examination and Vaccination Record, to one year from the date of submission to USCIS. Applicants must also submit Form I-693 to USCIS within one year of the immigration medical examination. USCIS will also provide additional ways to submit Form I-693. As outlined in policy alert PA-2014-005, this updated policy applies to any Form I-693 supporting a benefit application that USCIS adjudicates on or after June 1, 2014. - From USCIS
Attorney Brown has provided excellent advise and direction. As to the cost of such exams if they have to be redone, let USCIS take the lead on it and see if they will ask for a new one. I have seen it go both ways in Florida for a few days out but most follow the slippery slope theme that if 10 days is allowed then next 20 etc.... You can check around as it is often possible to get a better price of the med if you shop through the qualified dr list on the USCIS site for Florida. Good luck.
You will need another medical exam. It is not worth risking a denial of your application over a medical. I suggest going back to the same physician that did your previous medical and explaining that you need to update your medical. This way at least you know the physician has your immunization records. It could be faster and possibly reduce the cost of the new exam.
The Orlando Field office is generally pretty "by the book" so I would expect that if you show up with an expired medical examination, you will be told to send in a new one before a decision can be made on your case. If my client found him or herself in your situation, I would advise getting a new medical before the interview.

Yes, unfortunately if the medical exam expired at the time of your interview you will have to get a new one. However, in my experience, some doctors are willing to charge a nominal fee if they were the ones who performed the original exam. Ask your doctor.
