I was wondering what are the "Attachments" on FL-170.

I am filling out my final divorce forms and was wondering what forms these were on FL-170, Number 6d & 6d, Number 7a(3), and Number 8d. I was under the impression I could just use form MC-031 and label each attachment on there regarding our support, etc. but the court kicked it back. Are there specific forms for these so-called "Attachments" on FL-170? Thank you in advance!
