Are plea agreements guaranteed?

I have a relative who is currently incarcerated and has accepted a plea agreement in exchange for cooperation. Must the DA hold up their end of the agreement if the defendant has upheld on their behalf? He was supposed to be released on bail pending trial, they're not following with that stipulation. Who's to say they don't use them and leave them sit?

No, the judge must agree with the deal. And it is not likely the judge even knows of the proposal
