Is there anywhere I can obtain forms to create a pet trust for free?

I have been searching the internet for free forms to create documents specifying who will care for my cats in the event that I die or become incapacitated. I have come across sites like Rocket Lawyer and Legal Zoom to create these documents but the cost is approximately $50 depending on the options I choose. I am unable to afford this.

Although I strongly recommend that you consult an attorney (legal aid or a local ASPCA might be willing to help), if you must, go here for some assistance: http://www2.nycbar.org/pdf/report/uploads/8_20072453-ProvidingforYourPetintheEventofDeathHospitalization.pdf

Here's the website for Legal Aid of Orange County http://www.legalaidocba.org/ You could also create your own "Instructions-in-the-event-of incapacity-or-death" document. Make sure it's signed in front of 2 witnesses AND a notary.
