That depends on the attorney and her or his schedule. Call the lawyer and ask what the hold up is. I'm assuming this is your lawyer. If not then you just wait to receive notice and hire your own lawyer.

This depends on many factors. However, you should call and/or email your attorney to ask about the status of the petition. Be sure to follow-up your inquiry in a couple of days.
it would take forever as there is no such thing as full custody.
call the lawyer to see what is happening. if you do not like this lawyer, fire him or her and hire another.
It does not take a month to prepare a custody petition. Certain facts, which I assume you have given to your attorney, must be contained within the petition. I would give the attorney a reasonable amount of time (1-2 weeks) to get the petition to you for your signature. It should really only take about 30 minutes to an hour to prepare it. Call your attorney to see what the problem is. Your attorney could be ill and unable to work as usual for the time being, and/or engaged in countless emergency matters. If you otherwise like your attorney, let that person know that you want to have a good working relationship and to be kept advised of what is happening on your case. Reminders are always a good thing. That said, you should not have to hound your attorney to get things done.
