An estate planning attorney, among other things, can look at how the home is titled, what benefits your husband is currently receiving, and if any assets can be passed to your son without harming your husband's Medicaid eligibility.

Gifting of the home from father to son while father is on Medicaid will cause a substantial penalty for Medicaid. Father will lose his Medicaid benefits for a substantial period of time. You would either haveto take him out of the nursing home for the penalty period or pay the nursing home for close to the value of the house.
