"Guardianship" and "conservatorship" are two very different things. If you want to fully relinquish your parental rights, your mother would need to formally adopt your children, as local judges typically will not permit a parent to terminate their rights unless someone else is stepping up to adopt the kids. If you just want to make your mother a joint managing conservator with you, you'll need to ask the court to permit that to happen, and that will likely involve notifying the children's father of your plans. Contact a local family law attorney to assist you with this.

Signing over parental rights legally means executing an affidavit to facilitate termination of parent child relationship relationship so a child can be adopted. You may mean giving power of attorney to her and or giving her custody. Many times parents use giving up rights broadly. Go consult family law counsel.

It depends on where the other parent is and whether there is an existing court order. Meet with an attorney.
