Visitation modification

My ex and I agree to work together on visitation modification since I need to relocate from California to Nevada for a job offer. Can we write this agreement on plain paper and submit to the courts or does this have to be done on a certain form (FL-355/FL-341)? Do we have to attend a hearing?

You should do it on pleading paper. The California Rules specify how this should look. Most courts have pre-printed forms you can fill out in handwriting. You may also use the Judicial Council forms which apply. Make sure this addresses all the issues and you should include FC 3048. It is worth having a lawyer check it. t
