What forms are required to request a change in custody percentage?

Hello, we have had a court order for almost 6 years where I have 35% custody but I would like to have 50% custody. I can provide more details but I'm looking for what forms are required (beyond the OSC) to file for an increased custody percentage. I currently have 50% legal custody. Thank you very much! Dave

You need to file an RFO (fl300) and attach a declaration nad points and authorities. Consult with an experienced family law attorney to assist you with this.
