What are the bio-mom's rights to use my health insurance policy when I cover the stepkids and their dad under my insurance?

My husband and his ex-wife share 50/50 physical and legal custody in the state of Michigan. There is no primary parent or home and there is no custodial/non-custodial parent. Custody is exactly split down the middle. The mother does not work and lives with her boyfriend. Because she does not work, Michigan requires the working parent to provide health insurance. When I married, we chose to add him and the children to my health insurance. The FOC has been notified and it is perfectly legal, since my husband is also under my insurance. Since I am the policy holder, what are the mother's rights outside of medical records, attending appointments. etc.? Can she just use my policy however she wants? Doesn't the use of my insurance require our approval?

She has no right to use the insurance personally unless she is a named insured. If you are asking about hger using it for the children, then the answer would be when it is medically necessary for the children to have medical treatment and she has physical custody of the children

If the children are insured on your policy, the mother would have the right to use the policy for anything medically needed by the children. This would include appointments, testing, prescriptions, etc. She would not need your approval to do this. By placing the children on the policy, you have given the approval for them to use your policy. If the mother has questions about the policy, they will most likely not answer those questions, as she is not covered by the policy. She would have to get this information from you.
