A request for an amended report is fine, but you can also sit down with your personal injury attorney and fill out your own report to be filed. You should call your attorney and schedule a meeting. Good luck.

You can request an amended report if you have adequate proof. Go to the local PD which prepared the report and explain your proof, etc.

Request the police officer to file an amended report. No requirement that it be done. Be sure to report the incident to your insurance carrier.

You can file your own MV-104, I believe it is on the DMV website

in reality, police reports have absolutely no value as evidence as to how the accident happened. They are not admissible at trial as to how the accident happened as they are hearsay. If the officer that prepared the report actually saw how the accident happened it may come in, but usually this is not the case. I have a case where the police report was completely wrong, and I know this because I found video of the crash from a security camera after the report was filed.
