People forget to attach the exhibits to their legal documents all the time. It could have happened at any one of a number of places along the line of handling the document. This is not in and of itself fatal to the indictment. This answer is provided for informational purposes only. Sometimes there can be dismissals on technical grounds but there is an immediate refiling. Missing documents/logs always raise red flags in federal cases because prosecutors are not often sloppy. Is there an underlying problem? You haven't supplied enough facts for a more detailed answer.
