How do I fill out an emergency motion to adjust parenting time due to fear of safety of my children?

My name is Christopher Moring I have my 3 children full time. My ex in the last few months has tried to leave her husband multiple times and keeps going back to him. In that time he has filed emergency orders of protection against her and filed for divorce. 2 days after he filed for divorce she went back again. She has told me multiple times that hes verbally mentally and emotionally abusive. She has told me that he takes their prescription pain medication and sells them for extra money. Also they are being evicted for the 3rd time. He has pinched my oldest child's nipple and left a bruise. my youngest child has told me "Ron (my ex's husband) is a butt head because he always pinches us" He has no valid drivers license in the state of Illinois and my children tell me repeatedly that he drives everywhere. Shes Currently going to counseling for her anxiety due to his abuse. currently there's a temporary parenting agreement that she gets our 3 children every other weekend and she can come see them on wed for 4 hours. She has never once came on a wed to see them. My main concern is keeping my children safe and away from the unstable environment that her husband creates.
