In addition to Mr. Chen's answer, I wuold only add: Assuming this property was awarded to the wife as her separate property, the ex-wife needs to file an Order to Show Cause (OSC) in the same court that the divorce was entered, under the same case number, invoking the court's retained jurisdiction to divide the community assets and enforce the terms of the judgment. Ex-wife should consult with a family law attorney. Hopefully, a letter from an attorney will get things moving. If ex-husband's lack of cooperation is unjustified, the court can sanction him for the cost of ex-wife's attorney fees.

The ex husband would have to provide a quitclaim deed to the ex wife, showing he has no claim in the property.
Actually, the original title company which insured the title when the ex wife aquired the property as her sole and separate property should have obtained the quitclaim deed from the husband at that time.
