The form I-20A-B with entry stamp should be sufficient evidence of lawful admission to the United States allowing the USCIS officer to complete adjudication of your form I-485. However, if the USCIS officer is unwilling to complete the adjustment based upon the stamp, as a precaution, you should file a form I-102 to obtain a 'new' copy of your form I-94. Finally, you need a new passport to travel.

Possibly, but you should should file an I-102 immediately to replace the I-94. That way you can show that you have taken all the steps necessary to replace the I-94.
