Application Form DS-11 for the US Passport No.4 asks the Place of Birth. Is the Place of Birth the Place of Birth Hospital or Residence? The city of the residence may be different from the city where the hospital is though they are in the same state. How does the law define the Place of Birth?

Place of birth is the physical address of where you were born. For example, a person whose parents were from Montreal Quebec could have had a child when they were visiting Toronto, Ontario. The place of birth is Toronto, Ontario. Hope this helps.
