For i-485: do you need to provide copies of ALL pages of a passport or only those containing visas (or entry/exit stamps).

What if the visas are expired, do you still need to copy? What about expired passports, do you need to provide copies of those as well?

You should read through the instructions or consult with an attorney.

As with most things legal, it depends. If there is any question as to your legal status, USCIS will want to see the entire passport. If there are no questions, biographic page, visa page, and I-94 should suffice. You should retain an experienced immigration lawyer to review all the facts and advise you accordingly.
