Nobody can tell you based on a post to whether you have a sound basis for a lawsuit. Given the nature of the injury you describe it would certainly be worth your while to consult with one or more lawyers. This is not strictly speaking a medical malpractice case, but since it involves issues of health care, a medical malpractice lawyer would be the most appropriate specialty to review the case. There are some very good medical malpractice lawyers in the Albany area. If you have any difficulty locating a lawyer to review your matter, contact me for a recommendation. Do not delay, the statute of limitations may be a problem.
My law partner is an Emergency Medical Service expert having served as a chief of the local EMS for over a decade. He does not believe it is possible that you to anything in your thoracic area, but more likely your esophagal region. Such tears are not uncommon in the field (as opposed to a hospital) and whether it is negligence will depend on a lot of factors that can only be addressed from a full review of the record. In a hospital setting it is more likely to be considered malpractice. You need to have your file reviewed by a an emergency medical expert and/or lawyer who handles these cases or preferably by a lawyer like my partner who is both. You need to act quickly.
