Hello, my cousin has asked me to sponsor him for a visitor's visa via form I-134 (affidavit of support). My question is: where do I send this form? The form gives no instructions. My cousin will be interviewing in the Mumbai consulate.

You send it to your cousin. He takes it with him to his interview.

I agree with my colleagues. As a sponsor you should send the document to the applicant, and let applicant should carry those documents to the consulate for the interview.
