I have not filled out I-94 on the airplane again without knowing it should be done each time. Anyone knows what to do please?

This made it look as if I enetered the country illegally, which was not possible because I was coming via airplane. My first I-94 was thrown out by a careless clerk of the school and second I94 was not filled out by me on the way in to US second time. It has been long time and I don't know how I should solve this problem by now. I want to hear from someone. Thank you.
