This does not sound like the kind of thing there is a pre-printed form for. An affidavit is a sworn statement. You can find a similar TYPE of form and draw up your own. Without knowing more, I am not sure whether you need a lawyer or not. A lawyer could draft an affidavit in about 10-15 minutes. Whether it would be worth it for you to pay to have this done or not is not clear. James Frederick

It may depend on the reason for the affidavit as to whether it needs to be formally drafted and notarized or prepared by an attorney, or it may be as simple as handwriting an attestation with a signature and date. Please feel free to contact me and I would be happy to discuss further with you. Douglas R. Coenson, Esq. [email protected] (561) 315-2120 Jupiter, FL

The party requiring the affidavit may have a form you can use. There are many forms for an affidavit online. You need to include the representation that you don't own a motor vehicle. If you use a form make sure you do not include any statements from that form that are not relevant to your affidavit. Remember to sign in front of a notary. If you go to you own bank they will usually do this for free. Other notaries will likely charge $5-$10.
