You really need to sit down with an experienced family law attorney and discuss your case with them. It sounds like there is a lot of the story missing.

The Maricopa County Superior Court Self-Service Center has a number of forms and instructions for the public to use. You can find the site through www.superiorcourt.maricopa.gov. Just follow the link for Juvenile Cases to see the sampling. If you want to bring a state-law civil case for damages against the State of Arizona for CPS actions, you would need to file a Notice of Claim, under A.R.S Sect. 12-820.01 within 180 days of the date of injury. You can find sample Notice of Claim forms at www.azrisk.state.az.us. The rules for the Notice of Claim can be complicated. You would be wise to consult with an attorney.
