Is the California Form CIV-50 STATEMENT OF DAMAGES the appropriate form to use in a breach of contract case?

On this form (CIV-50) it has parenthesized "personal injury" or "wrongful death". If my case is a contract for goods/services case is this the correct form for me to use or is there a form specifically for contract cases that I must mail to defendant? THANK U ANY 1

A statement of damages need not be prepared and served in a breach of contract case. You can do it if you want to, but it is not required.

There's a form you can use to sue a defendants for breach of contract (PLD-C-001 (1), and also one that's used for services (PLD-C-001 (2). I suggest you spend an hour with a lawyer, to get your case off on the right foot. It's only going to get more complicated from here, and you can't expect to use this site to teach you how to litigate.

It is unclear whether you are referring to what you need to file the case or whether this is related to discovery. This is why Ms. Koslyn is right. You should contact an attorney to help you on this because it's only get more complicated after this.

If you are trying to file a civil case for breach of contract for the first time, use the following forms: http://www.courts.ca.gov/documents/pldc0011.pdf http://www.courts.ca.gov/documents/pldc001.pdf http://www.courts.ca.gov/documents/sum100.pdf http://www.courts.ca.gov/documents/cm010.pdf Then file with the court and then serve the defendant. I hope this helps.
