My permanent residence card expired 3 years ago I had to go back to Italy my family needed me I am now back how can I fix this

You may have abandoned your Permanent Resident status, especially if you were out of the country for 3 years, and re-entered as a tourist. More information would be needed about the reason you spent so much time abroad, if you maintained any ties in the United States, and/or if you took steps to preserve your residence through filing a re-entry permit.
