How do I complete a application and order for appointment of guardian ad litem-civil ex parte?

On the form Civ-010 for Application and order for appointment of guardian ad litem-civil ex parte on page 1 on #1 where is says since I am the parent I put my name and underneath it I checked the parent of and put my child name. My child is actually 14 now so should I also check E. also that states the minor to be represented (If the minor is 14 years of age or older).
