Payer of child support is 3 months behind. Is the "contempt of court" petition the form I should file?

Haven't received any child support for 3 months, and he hasn't paid 1/2 of uncovered medical costs in 3 years( but was ordered to). Do I simply fill out the contempt form and state the facts stated here, or should a attachment of facts also be listed?

It sounds as if a simple contempt motion would be appropriate. However, you should consult with an experienced family law attorney just to make sure. Or, you could also ask the Child Support Enforcement Agency in your county to assist you in this. The entire purpose of the agency is to assist people in collecting support. They will do this at no charge after payment of a one-time fee, which varies by county (usually about $50), to open an case with them. I have attached a link with their information.
You might want to call your local child support enforcement office. At four(4) months of non-payment they get very testy. It is a crime by statute, and they will enforce it. Call them first, because they won't charge you to do so.
In addition to the previous answers, you may also want to contact your local District Attorney's office . In some counties, they are pursuing felony charges if the payor fails to pay support for too long and all avenues in family court have been exhausted.
