Is there a particular format I need to follow on a Request for Production of Documents I plan to serve on a plaintiff?

It is for a civil case in CT with me as a defendant against a debt collector. Section 13-9 of the Practice Book lists some Forms that can be used for these requests. Am I obligated to use one of these forms?
